The LIFE program, or Living Independently
For Elders, is a unique
healthcare opportunity not only for the IHM Sisters, but for
the local elderly population in the Scranton, Pennsylvania,
area.
Known as PACE in other states, the LIFE program provides all
necessary medical and supportive services to the Sisters,
as well as to those individuals from the neighboring community
who are eligible to receive services. LIFE services offer
adult day-care, consisting of nursing, physical, occupational
and recreational therapies, meals, nutritional counseling,
and personal care. The LIFE model is a model of choice for
older individuals focused on staying at home and aging in
place.
The LIFE program serves individuals who are age 55 and older,
certified by their local Area Agency on Aging to need nursing
home care, are able to live safely in a community at the time
of enrollment, and reside in a geographic location assigned
to the LIFE provider. LIFE services also include access to
healthcare 24 hours a day, 365 days a year. In addition, a
LIFE physician familiar with the history, needs and preferences
of each participant provides medical care.
Overall, the LIFE program allows seniors to keep their independence
while helping to provide them with necessary care.
